Submitting a Discrimination Complaint

To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online here and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Hours

Our main hospital lobby hours are 7 a.m.-5 p.m., Monday-Friday
Lab Hours for walk-ins are 7 a.m.-5 p.m., Monday-Friday
Health Clinic hours are 8 a.m.-5 p.m., Monday-Friday
Business Office hours are 8 a.m.-4 p.m., Monday-Friday
Emergency Department hours are 24/7 – please enter through the ED doors

Visiting Hours

Please speak with a member of the nursing staff before visiting a patient to determine whether your visit may require special arrangements. As Southern Coos Hospital & Health Center joins the rest of the country in responding to COVID-19, we have reduced visiting hours are from 12pm to 4pm. These hours will be in effect until further notice. 

Visiting Protocols

Quality care is important to Southern Coos Hospital and Health Center. To provide patient safety and recovery, the following protocols have been established:

  • Wash your hands before entering and leaving patient rooms.
  • Cover coughs and please utilize the hand sanitizer dispensers located throughout the hospital.
  • Visitors who have symptoms or who have been exposed to infectious disease within the last three weeks please speak with a member of the nursing staff before visiting.
  • Limit the use of strong perfumes and scented lotions to prevent reactions or further illness.
  • Keep noise low to support patient recovery.
  • Respect patient confidentiality and be aware of posted signs.

Smoke Free Campus

We are a Smoke Free campus. Smoking is not allowed in the building or on the facility property including the parking lot. Please assist us in promoting good health habits by not smoking on the hospital premises.

Information

The Patient Services Coordinator and Admitting desks are located at the main entrance, staffed by knowledgeable personnel who can assist you.

Refreshment

Coffee and water are available to visitors and patients 24 hours a day. Please check at the nurse’s station for assistance. Vending machines are also available for snacks.

Gift Shop

The Gift shop, volunteer operated, is located in the lobby Monday through Friday from 11 a.m.- 3 p.m. Proceeds from the gift shop help purchase patient-focused equipment for the hospital, support community health education programs and provide scholarships.

WiFi

Southern Coos Hospital and Health Center is pleased to offer free wireless Internet access for our patients and visitors. Our guest wireless network name is SCOOSGUEST, and is compatible with most wireless-ready computing devices. For your convenience, connectivity is available throughout the hospital main building. SCOOSGUEST is an unsecured connection. Southern Coos Hospital and Health Center assumes no responsibility, and shall not be liable for the loss of data, damages, or viruses that may infect your computer equipment or other property on account of your access to, use of, or browsing in any website, or your downloading of any materials from websites.

Medical Record Requests

To obtain a copy of your medical records, please begin by completing this Medical Record Authorization Form form and returning it to Southern Coos Hospital & Health Center.
